COVID Symptoms
Once you complete this form, someone from the school will be in contact with you. Please do not send your child to school until someone has been in contact with you.
A message from SDPC:
Students who have any signs of illness must stay home. Students who are being tested for COVID cannot come to school while waiting on test results. If a student tests positive, complete the COVID reporting form. If there is someone in the home that has tested positive or a student has been in close contact with someone who has COVID, the student cannot come to school. Complete the COVID Reporting Form and wait for someone from the school to contact you.
